Significance and method of use
	
	
		
Traffic accident certificates are issued on request from the parties involved in an accident. They state the date and time of the accident, the names and addresses of the parties involved, and other relevant details.
		 JSDC certifies these details based on data kept by the police. Among others, these certificates are used as supporting documents when claiming various kinds of automobile insurance benefits.
		 The certificates are prepared using personal information held by the police. As such, applications are only accepted from a party  involved in a traffic accident (either at-fault or non-fault), or someone applying on their behalf by proxy.
	 
 
Application method and certification fees
Applications may be made via transfer from any post office, using an application form (transfer form). Forms may be obtained at JSDC offices, police stations, non-life insurance companies, etc. Also, you can apply directly to your nearest JSDC office.
 The fee is 800 yen per certificate.
